BACKGROUND: Understanding the epidemiological trends of polycystic ovary syndrome (PCOS) and related-infertility among women of childbearing age (WCBA) is crucial for developing effective public health strategies. This study aims to analyze epidemiological trends of PCOS and related infertility among WCBA using Global Burden of Disease (GBD) study 2021.

METHODS: Using GBD Study 2021, this study examined trends of PCOS and related infertility among WCBA across global, age, regional, national, and socio-demographic index (SDI) levels from 1990 to 2021, and projected the prevalence to 2050. Key trends were assessed via Joinpoint regression, and future prevalence was projected using a Bayesian Age-Period-Cohort (BAPC) model.

RESULTS: In 2021, the global prevalent cases of WCBA affected by PCOS and related infertility were 65.77 million and 12.47 million, respectively, with prevalence rates of 3,374.68 per 100,000 population and 639.63 per 100,000 population. The infertility percentage among those with PCOS was 18.95 %. From 1990 to 2021, the prevalence rate of PCOS increased overall (average annual percentage change [AAPC] = 0.85), with significant increases during 2010-2021 (annual percentage change [APC] = 1.06). PCOS-related infertility also increased significantly during 2011-2021 (APC = 1.24). The AAPC in the prevalence rate of PCOS and related infertility varied markedly across regions, countries, age groups, and SDI levels. Regionally, the largest increases were observed in Southeast Asia, South Asia and East Asia. Nationally, the largest increases were in Equatorial Guinea, Maldives, and Myanmar. By age group and SDI level, the largest increases were in 20-24 years age group and the middle SDI levels. The BAPC model projects a sustained upward trajectory in the prevalence of PCOS and PCOS-related infertility.

CONCLUSIONS: In recent years, the growing burden of PCOS and related infertility among WCBA emphasizes the urgent need for tailored strategies to address disparities across region, nation, sex, and SDI levels and mitigate future impacts.
