OBJECTIVE: To study whether omitting endocrine and ultrasound monitoring in hormone replacement therapy (HRT) cycles for frozen embryo transfer (FET) affects pregnancy rates in good-prognosis patients.
DESIGN: This retrospective cohort study included 294 patients who underwent single, frozen-thawed blastocyst transfer in HRT cycles between June 2023 and October 2024 at Bahçeci IVF Center in Istanbul, Turkey. Patients were stratified into two groups: study group (no monitoring) and control group (ultrasound and hormonal monitoring).
SUBJECTS: Women aged <37 years with primary infertility because of unexplained infertility, polycystic ovary syndrome, mild male factor infertility, or a combination of the last two.
EXPOSURE: Hormone replacement therapy endometrial preparation with or without endocrine and ultrasound monitoring.
MAIN OUTCOME MEASURES: Ongoing pregnancy rate per embryo transfer. Secondary outcomes included implantation and miscarriage rates.
RESULTS: Pregnancy rates were 68.4% (study group) and 69.7% (control group) (Odds ratio [OR]: 0.94, 95% confidence interval [CI]: 0.53-1.65). Implantation rates were 57.9% and 62.4%, respectively (OR: 0.82, 95% CI: 0.48-1.41). Ongoing pregnancy rates were 46.1% and 49.1%, respectively (OR: 0.88, 95% CI: 0.52-1.49). Miscarriage rates were 11.8% and 13.3%, respectively (OR: 0.87, 95% CI: 0.39-1.94). Monitoring was not an independent predictor of ongoing pregnancy (adjusted OR: 0.85, 95% CI: 0.49-1.47).
CONCLUSION: Omitting endocrine and ultrasound monitoring in HRT cycles for FET does not impair pregnancy outcomes in good-prognosis patients undergoing autologous frozen-thawed embryo transfer after a freeze-all protocol. This practice minimizes patient discomfort and costs without compromising pregnancy outcomes.
