ETHNOPHARMACOLOGICAL RELEVANCE: Depression is a common psychiatric disorder and has become a growing public health issue. In the traditional Chinese medicine (TCM) clinic, blood deficiency is a common syndrome pattern in depression. Xiaoyao San (XYS), a classic formula with the property of nourishing blood, is widely used to treat depression. However, how the blood-nourishing action of XYS contributes to its antidepressant efficacy and what its underlying mechanisms are remain unclear.

AIM OF THE STUDY: The current study aimed to investigate the mechanism by which XYS exerts its antidepressant effects through the enhancement of blood-nourishing property.

MATERIALS AND METHODS: Network pharmacology methods were employed to predict the key pathways of XYS in the treatment of depression and blood deficiency syndrome. Chronic unpredictable mild stress (CUMS) model was employed to assess the therapeutic effect of XYS on depression-like behaviors accompanied by symptoms associated with blood deficiency in TCM. Lipidomics, western blotting, and histological techniques were integrated to explore the underlying mechanisms. Molecular docking simulations validated the binding affinity between core targets and XYS's active compounds.

RESULTS: Network pharmacology analysis reveals that XYS targets common pathways in the management of depression and blood deficiency syndrome, particularly involving HIF-1 signaling pathway and lipid metabolism-related pathways. Animal experiments demonstrates that XYS has the potential to ameliorate depression-like behaviors and the symptoms associated with blood deficiency induced by CUMS modeling. Lipidomics results showed that the perturbations in lipid metabolism were effectively reversed following treatment with XYS, particularly impact sphingolipid metabolism. Concurrently, mechanistic analysis revealed that XYS mediates its effects on depression by modulating targets in sphingolipid and HIF-1 signaling pathways. Molecular docking identified stigmasterol, cubebin, β-sitosterol, and xambioona as potential therapeutic agents for XYS therapy due to their strong binding affinities with SMPD1 and HIF-1α.

CONCLUSION: XYS exerts anti-depressant effect by enhancing its blood-nourishing property, which are associated with the inhibition of the sphingolipid metabolic pathway and the activation of the HIF-1 signaling pathway.
