Аннотация

The recent study by Kasahara et al., 'Risk-benefit stratification for immune checkpoint inhibitor therapy in head and neck cancer based on immune-related adverse events and treatment discontinuation using routine clinical indicators beyond PD-L1 expression', provides important insights into the relationship between skeletal muscle mass, immune-related adverse events (irAEs), and clinical outcomes in patients with recurrent or metastatic head and neck squamous cell carcinoma (R/M HNSCC) receiving immune checkpoint inhibitors (ICIs). This commentary highlights the significance of lumbar skeletal muscle index (LSMI) as a clinically accessible biomarker associated with both the development of irAEs and improved survival. The finding support emerging evidence that skeletal muscle functions as an active immunometabolic organ capable of influencing antitumor immune replies through muscle-derived cytokines and signaling molecules. The study further underscores the growing relevance of sarcopenia assessment in oncology, encompassing its utility beyond prognostic evaluation to the optimization of precision immunotherapy strategies. The proposed risk-scoring model incorporating age and lactate dehydrogenase levels offers a practical approach for identifying patients at increased risk of permanent ICI discontinuation. The clinical inferences of adopting host-related physiological indicators as complementary or alternative biomarkers to programmed death-ligand 1 (PD-L1) combined positive score, predominantly in settings where molecular testing is unavailable or limited. Its retrospective design and single-cohort population, it provides a strong rationale for future investigations integrating body composition, inflammatory markers, and immune profiling. The work advance understanding of the interplay between skeletal muscle health and immunotherapy efficiency and support the incorporation of sarcopenia assessment into personalized treatment planning for patients with R/M HNSCC.
