OBJECTIVE: To investigate the efficacy of concentrated growth factors (CGF) in the repair of oroantral communications (OAC), especially in the repair of bone tissue.

MATERIALS AND METHODS: Thirty patients with OAC defects measuring 3-5 mm were randomly assigned to either the CGF group (CGF repair, n = 15) or the control group (suture repair, n = 15). At 1, 3 and 7 days postoperatively, patients recorded pain intensity by a 10-point visual analogue scale (VAS). Soft tissue healing was evaluated using a modified version of the Mass Healing Index at 7 and 30 days postoperatively. The success rate of the oroantral communications defect closure was assessed at 30 days postoperatively. Cone-beam computed tomography (CBCT) scans were performed before and 90 days after surgery, while radiographic analyses were used to evaluate the height, volume, and density of new bone. Data were compared using t tests or Mann‒Whitney U tests, with statistical significance set at P < 0.05 for all analyses.

RESULTS: Patients in the CGF group reported significantly lower pain scores on Days 1 and 3 postoperatively compared to the control group (P = 0.049, P = 0.0122); however, comparable pain was observed in both groups at 7 days after the operation. The CGF group showed significantly better healing scores on Day 7 (p = 0.0148), while a healing effect was observed in both groups at 30 days postoperatively. Both groups achieved a 100% OAC closure success rate. The CGF group demonstrated superior new bone height, volume, and density (P < 0.05).

CONCLUSION: CGF use for OAC repair demonstrated benefits for soft and hard tissue healing in extraction sockets and postoperative pain, providing a simple, minimally invasive, and highly successful option for OAC repair following tooth extraction.

CLINICAL RELEVANCE: The CGF repair method provides a novel treatment approach for OAC with defects measuring 3-5 mm, particularly when implant restoration is necessary in the later stages of the lesion tooth area.
