Аннотация

OBJECTIVE: To examine the views of patients with ovarian cancer on biomarker (somatic) and genetic testing and existing patient-facing materials.

METHODS: We recruited patients with ovarian cancer for virtual focus groups and assessed patient experiences and knowledge of genetic and biomarker testing. We focused on themes of fear, knowledge, financial issues, and communication between patients and providers. Three team members coded focus groups using a thematic approach.

RESULTS: In October-November 2023, three focus groups were conducted, comprising seven participants with advanced ovarian cancer (mean 62 years; 85 % Non-Hispanic White, 15 % Asian). 85 % had undergone both genetic and biomarker testing. Participants displayed a stronger understanding of genetic testing, particularly BRCA, while biomarker testing knowledge was limited. Findings highlighted the importance of clinician communication in navigating genetic and biomarker testing, with participants emphasizing the need for guidance on the relevance of tests to treatment. Financial concerns, insurance issues, and confusion about the potential familial impact of biomarker testing emerged as barriers. Participants expressed a belief in the critical role of such testing in shaping treatment options and in familial risk assessment.

CONCLUSIONS: Patients identified several barriers to genetic and biomarker testing, including financial constraints, insurance coverage, and lack of clarity about the relevance of biomarkers to their treatment and familial risk. Conversely, a strong provider-patient relationship and clear communication were facilitators that supported patient engagement with testing (e.g., decision-making about testing, follow through with testing). Improving education on biomarker testing and offering targeted financial assistance may enhance patient understanding and access to these critical diagnostic tools in ovarian cancer care.
