ETHNOPHARMACOLOGICAL RELEVANCE: Hosta plantaginea (Lam.) Ascher flower is a traditional anti-inflammatory Chinese medicine, recognized for its therapeutic effect on benign prostatic hyperplasia (BPH). However, its mechanisms against BPH are still unclear.

AIM OF THE STUDY: This study aims to elucidate the active fractions of H. plantaginea flower responsible for anti-BPH effects and their underlying mechanisms.

MATERIALS AND METHODS: The chemical constituents in fractions of H. plantaginea flower were rapidly identified using UHPLC-MS. The anti-BPH efficacies of the effect fractions were evaluated in testosterone propionate (TP)-induced RWPE-1 cells and rat models. Metabolomics and bioinformatics analyses were used to identify potential metabolic and signaling pathways, which were validated by Western blotting.

RESULTS: A total of 26, 38, and 23 phytochemicals were fleetly identified from petroleum ether (HPA), ethyl acetate (HPB), and butanol (HPC) fractions, respectively. The HPA, HPB, and HPC fractions of H. plantaginea flower exhibit significant anti-BPH effects at equivalent doses of the raw H. plantaginea flower herb (1 and 4 g/kg), including reductions in prostate enlargement, wet weight, and prostatic index, along with improvements in damaged prostate tissue. Additionally, these fractions significantly increased testosterone levels and decreased dihydrotestosterone, 5α-reductase, and epidermal growth factor levels in prostate tissue. In addition, HPA and HPB fractions possess anti-BPH activities in vitro. Untargeted metabolomics revealed that HPA, HPB, and HPC reversed 33 differential metabolites involved in amino acid metabolism and the regulation of AMPK/JNK pathways, as well as their downstream STAT, PI3K-Akt, MAPK, and NF-κB pathways. Mechanistically, HPA, HPB, and HPC significantly inhibited the overexpression of phosphorylated proteins, including AMPKα1, JAK1, STAT3, PI3K p85, Akt1, JNK, p38, Erk, p65, and IκBα.

CONCLUSIONS: H. plantaginea flower exhibits significant anti-BPH effect by inhibiting the AMPK/JNK signaling pathways and regulating amino acid metabolism. HPA, HPB, and HPC were identified as the major effect fractions, with fatty acids and flavonoids as potential pharmacodynamic substances. These findings offer a scientific basis for the clinical application of H. plantaginea flower in treating BPH and provide groundwork for developing BPH treatments based on this herb.
