Аннотация

OBJECTIVE: To describe the clinical findings and outcomes in dogs diagnosed with thoracic or pelvic limb ischemic neuromyopathy (INM) in a referral practice.

METHODS: This was a retrospective, multicenter study of client-owned dogs diagnosed with thoracic or pelvic limb INM at 5 referral hospitals (July 2013 to June 2023). Medical records were reviewed for clinical findings, comorbidities, treatments, and outcomes. The primary outcome was survival to discharge. Long-term outcomes were summarized when follow-up was available.

RESULTS: 42 dogs met the inclusion criteria: 4 of 42 (9.5%) had thoracic limb INM, and 38 of 42 (90.5%) had pelvic limb INM. Comorbidities were documented in 35 of 42 (83.3%), most commonly protein-losing nephropathy (21 of 42 [50.0%]). All thoracic limb cases were ambulatory at presentation but were euthanized shortly after diagnosis at the owner's discretion. Among pelvic limb cases, 22 of 38 (57.9%) were ambulatory at presentation, 27 of 38 (71.1%) retained nociception, and 15 of 38 (39.5%) had onset of clinical signs > 24 hours. Nineteen of 38 (50.0%) survived to discharge. Among dogs with long-term outcome available, 7 of 28 (25.0%) fully recovered, including 2 of 16 (12.5%) initially nonambulatory dogs.

CONCLUSIONS: In dogs with pelvic limb INM, ambulatory status, presence of nociception, and more gradual progression to maximal clinical severity were associated with survival to discharge. Complete recovery occurred in a subset of initially nonambulatory dogs, indicating that recovery is possible despite more severe neurologic deficits at presentation.

CLINICAL RELEVANCE: Prognostic indicators such as ambulatory status and nociception should be used to inform clinical decision-making but should not be used in isolation when considering prognosis, as recovery may occur even in more severely affected dogs.
