Monitoring and improving the effectiveness of cleaning medical and surgical devices: A practical verification framework.
Аннотация
BACKGROUND
Cleaning is the foundational step in medical device reprocessing; residual soil and biofilm can compromise disinfection and sterilization and contribute to infections. Despite the importance of cleaning, healthcare facilities often rely on visual inspection, which may not detect residue inside lumens. This article examines evidence-based approaches to monitoring cleaning, with emphasis on implementation strategies for infection preventionists.
METHODS
Published literature on cleaning monitoring and standards for medical device reprocessing were reviewed. A practice-focused framework was developed, emphasizing people, equipment, and monitoring strategies mapped to workflow points throughout the reprocessing cycle.
RESULTS
A reliable cleaning program requires visual inspection with lighted magnification, internal inspection of lumens using borescopes, and biochemical tests. Studies demonstrate that debris can persist in lumens despite standard cleaning processes, and outbreak investigations underscore consequences of inadequate monitoring. Emerging technologies including artificial intelligence-assisted borescope interpretation and automated tracking systems are transforming cleaning quality assurance from subjective to objective, data-driven monitoring.
CONCLUSION
A structured monitoring strategy paired with competency assessment, equipment monitoring, and data-driven quality improvement reduces reliance on assumption and strengthens defensible practice. Infection preventionists are essential bridges between clinical and technical departments, integrating data into quality management systems to improve cleaning reliability and patient safety.
