Surgical Management of Severe Retinal Fibrosis and Regaining Vision following Suprachoroidal Hemorrhage in a Negative Light Perception Patient: A Case Report.
Аннотация
PURPOSE: To report a rare case of vision regained in an eye with no light perception (NLP) secondary to severe ocular trauma and suprachoroidal hemorrhage (SCH) following a syncopal fall on a sharp object.
METHODS
A 75-year-old one-eyed man presented with NLP vision and a funnel-shaped retinal detachment with residual slit remnants of SCH on B-scan ultrasonography. The patient was informed that the aim of the surgery was to prevent phthisis and preserve the eye's structure. Careful and meticulous dissection of the fused retinal tissue was performed, followed by a 360° retinectomy to achieve retinal flattening, and the procedure was concluded with silicone oil tamponade.
RESULTS
The retina was fully reattached intraoperatively. Visual acuity improved from NLP preoperatively to counting fingers at 1 meter one month after surgery. The globe remained formed, and the retinal reattachment and visual function remained stable on follow-up.
CONCLUSION
Even eyes with NLP vision after severe trauma and SCH may regain ambulatory vision with advanced vitreoretinal approach. This case report underscores the importance of the surgeon's utmost skill and surgical expertise in restoring both the anatomical integrity and the functional rehabilitation.
