[Qualitative comparative analysis of high-risk configurations of occupational stress, burnout and insomnia among primary medical and health personnel in a certain area of Hebei Province].
Аннотация
Objective
To understand the current situation of occupational stress, burnout, and insomnia among primary medical and health personnel in a certain area of Hebei Province, and the qualitative comparative analysis (QCA) was used to frame the high-risk population configuration for occupational stress, burnout, and insomnia among primary medical and health personnel, providing a basis for precise intervention.
Methods
In August 2021, all employees of 7 public hospitals and 8 public health institutions in Xiong'an New Area were selected as survey objects. Questionnaire survey was conducted using the general situation questionnaire, Effort Reward Imbalance Scale (ERI), Maslach Burnout Inventory-General Survey (MBI-GS), and the Ashens Insomnia Scale (AIS). The chi-square test was used for univariate analysis, and the meaningful variables in the univariate analysis were included in the QCA to analyze the configuration of high-risk factors of occupational stress, burnout and insomnia among primary medical and health personnel.
Results
2675 valid questionnaires were collected, with an effective response rate of 88.63% (2675/3018). The overall incidence of occupational stress among primary medical and health personnel was 51.48% (1377/2675), including 58.53% for doctors, 59.54% for nurses, and 31.14% for public health personnel. The incidence of burnout was 57.12% (1528/2675), including 59.00% for doctors, 63.95% for nurses, and 49.14% for public health personnel. The incidence of insomnia was 56.75% (1518/2675), including 59.72% for doctors, 62.07% for nurses, and 38.86% for public health personnel. Not drinking alcohol or occasionally drinking alcohol was a necessary condition for primary medical and health personnel to experience occupational stress, the consistency value (C) was 0.951. The configuration analysis results showed that the high-risk factors configurations of occupational stress were mainly manifested as high effort type and low return and no exercise type. The high-risk factor configurations for burnout were low personal achievement type and high occupational stress type, while the high-risk factor configurations for insomnia were high economic burden and high internal investment type and high occupational stress type. The overall consistencies of the configurations were 0.864, 0.828 and 0.914, respectively.
Conclusion
The occupational stress, burnout, and insomnia of primary medical and health personnel in Hebei Province cannot be ignored, and their occurrence is the result of multiple influencing factors. The QCA method can frame the configuration of high-risk factors and provide a basis for precise intervention.
