Abstract Topic: 30. Infections in hematology (incl. supportive care/therapy)

Background: Haematological malignancies (HM), also known as haematological neoplasms, are tumours that originate from the blood and blood‐forming system (marrow and lymphatic tissue), including leukaemia, lymphomas and multiple myeloma. With nearly 920,000 new cases of HM each year, HM has become a common malignancy worldwide. HM patients may be hospitalized due to immunodeficiency, disease progression, intensive chemotherapy, or adverse effects from treatment. Unplanned hospital readmission cause the HM patients physical and mental stress, prolong hospitalization times, increase treatment costs, and overburden healthcare resources, so they are receiving increased attention and the 30-day unplanned hospital readmission rate is increasingly regarded as a measure of hospital quality of care. So early identification of high-risk HM patients and undertaking timely preventive measures are therefore very important. However, there are few studies on the incidence and predictors of 30-day unplanned hospital readmission among HM patients in China.

Aims: To measure the incidence of 30-day unplanned hospital readmission in HM patients in China, and explore the related factors for 30-day unplanned hospital readmission.

Methods: This cohort study examined patient data were collected using the hospital electronic medical system of the Institute of Hematology & Blood Diseases Hospital, Chinese Academy of Medical Sciences & Peking Union Medical. All adults (age ≥18 years) who patients who received the wrong treatment in hospital between January 4, 2017, and November 1, 2020, were included. The analysis was performed from September 2022 to February 2023. The main outcome was 30-day unplanned hospital readmission rates for HM partients. Factors associated with unplanned hospital readmission, including demographic characteristics and disease-related characteristics, were measured.

Results: There were a total of 1,252 patients with a median age of 50 years. The 30-day unplanned hospital readmission rate was 18.45%. The results showed that HM disease patients with different ages, disease diagnoses, pulmonary infection, hospitalization days, BMI, ECOG, ALB had statistically significant different 30-day unplanned hospital readmission (p <0.05, Table 1). Binary logistic regression analysis showed the strongest predictors of 30-day unplanned hospital readmission were lymphomas (OR=1.527, 95% CI: 1.127-2.069), hospitalization days (OR=0.989, 95% CI: 0.979-0.999), ECOG (OR=1.192, 95% CI: 1.037-1.370), ALB (OR=0.962, 95% CI: 0.941-0.983), Table 2.

Summary/Conclusion: It is suggested to focus on the 30-day unplanned hospital readmission among HM Patients with lymphomas, shorter hospitalization days, poor physical activities, and low ALB, because they have high risk of 30-day unplanned hospital readmission. These results may also provide guidance when developing quality indicators and policies penalizing hospitals for HM Patients.
