BACKGROUND: The acoustic analysis of substitution voices presents significant challenges. Traditional acoustic indices often fail to provide reliable assessments due to the intrinsic irregular nature of these signals. The present study aims to evaluate the validity of various acoustic parameters, including Cepstral Peak Prominence Smoothed (CPPs) and the Acoustic Voice Quality Index (AVQI), alongside traditional measures such as Jitter (Jitt%), Shimmer (Shimm%), and Harmonic to Noise Ratio (HNR) for assessing the quality of substitution voices after open partial horizontal laryngectomy (OPHL).

METHODS: Sixty-six patients who underwent OPHL types II and III were retrospectively analyzed. For each patient, voice samples of sustained /a/ vowel phonations (SV) and continuous speech (CS) were recorded. These samples were subjected to both auditory-perceptual assessments by expert clinicians and acoustic analysis using CPPs, AVQI, HNR, Jitt%, and Shimm%. The acoustic parameters were calculated for the SV samples and the concatenation of SV and CS samples. The severity of voice quality was evaluated using the I subscale from the INFVo scale, and a comparative analysis of acoustic measures across perceptual severity classes was performed.

RESULTS: A strong correlation was found between auditory-perceptual ratings and the parameters CPPs (r = -0.77), and AVQI (r = 0.69). Traditional acoustic measures like Jitt%, Shimm%, and HNR showed weaker correlations with perceptual assessments, especially when calculated for SV alone. The analysis demonstrated that CPPs, calculated on the CS+SV concatenation, provided the most reliable differentiation between mild, moderate, and severe voice alterations, followed by AVQI. Diagnostic accuracy of CPPs and AVQI was calculated as well, showing good performance for both the parameters.

CONCLUSIONS: This study suggests the reliability of CPPs and AVQI as promising acoustic parameters for evaluating substitution voices after OPHL. Integrating these measures into a standardized multidimensional protocol for substitution voice assessment could enhance diagnostic precision and treatment outcomes for surgical and speech therapy interventions.
