OBJECTIVE: To investigate the influence of donor age and sex on the efficacy of red blood cell (RBC) transfusion in patients with chronic hematological diseases.

METHODS: A retrospective analysis was conducted on 222 patients with chronic anemia who received RBC transfusions at the Affiliated Hospital of Southwest Medical University from January 1 to December 31, 2023. The increments in hemoglobin (Hb) and hematocrit (HCT) (δHb and δHCT) were compared between recipients who received RBCs from donors of different sexes and between those who received RBCs from male donors of different ages, so as to evaluate the transfusion effectiveness.

RESULTS: Recipients who received RBCs from male donors had a higher δHb than those who received RBCs from female donors (P <0.001). Recipients who received RBCs from male donors showed an increasing trend in δHCT compared to those who received RBCs from female donors, but the difference did not reach statistical significance (P >0.05). Further comparison among recipients who received RBCs from male donors revealed that those who received RBCs from donors aged <30 years had a significantly higher δHb than those who received RBCs from donors aged≥30 years (P <0.001). While recipients in the younger (<30 years) male donor subgroup showed an increasing trend in δHCT, but the difference relative to the older (≥30 years) male donor subgroup was not statistically significant (P >0.05). Results of regression analysis revealed that the age of blood donors exerts a negative impact on δHb; that is, the older they are, the lower the δHb. For blood donors or recipients, males show a higher δHb than females (P <0.001), while the effects of these factors on δHCT were not statistically significant (P >0.05).

CONCLUSION: Transfusing RBCs from young male donors is more beneficial for increasing Hb levels in recipients.
