Аннотация

BACKGROUND: Critical care medicine is essential for managing life-threatening illnesses, yet in many low- and middle-income countries (LMICs), access remains limited by shortages of trained specialists and infrastructure. In Rwanda, only three locally trained critical care physicians were available in 2025, underscoring an urgent workforce gap. In response, national leadership envisioned a locally based fellowship program to train intensivists and reduce reliance on external pathways.

METHODS: A competency-based curriculum was developed by the University of Rwanda's College of Medicine and Health Sciences in partnership with the Ministry of Health and international collaborators. The design adapted international standards to Rwanda's resource-constrained context, incorporating clinical rotations, simulation training, research, and specialized modules in echocardiography, nephrology, cardiology, and the Critical Care Consortium. The curriculum underwent multiple institutional reviews and was formally accredited by the Higher Education Council in 2025.

RESULTS: Six fellows were recruited into the inaugural cohort and placed across four national referral hospitals. A structured teaching program was established, including weekly journal clubs, local and international seminars, and access to e-learning platforms. Faculty appointments combined local specialists with honorary international lecturers. The program officially launches in September 2025, beginning with induction and the first year of clinical rotations.

CONCLUSION: The Adult Critical Care Fellowship represents a landmark in Rwanda's health workforce development. By moving from need recognition tovision, from vision to action, from action to accreditation and from accreditation to implementation, Rwanda has created a replicable model for LMICs seeking to strengthen critical care capacity through locally led, accredited training.
