Development of a web application for the Surgical Risk Preoperative Assessment System (SURPAS).

BACKGROUND

The Surgical Risk Preoperative Assessment System (SURPAS) is a parsimonious surgical risk calculator using 7 preoperative variables to predict 15 postoperative complications for >3000 operations across 10 surgical specialties. Currently only available at our local healthcare system (UCHealth), this study reports on the development of a publicly-available SURPAS web application.

MATERIALS AND

METHODS

The SURPAS web application was developed as an R Shiny App. Current SURPAS usage within UCHealth, 2017-2023, including patient characteristics and risk profiles, are provided with descriptive statistics.

RESULTS

SURPAS was used locally for 24,431 operations; increasing from usage in 0.5 ​% of operations in 2017 to 10.0 ​% in 2023. It has been used in ten surgical specialties for mostly inpatient and non-emergent procedures. Usage has increased in all specialties. Average 30-day mortality and overall morbidity risks were 1.6 ​% and 13.8 ​%, respectively.

CONCLUSIONS

Dissemination of the SURPAS web app could increase SURPAS usage inside and outside of UCHealth, augmenting communication of complication risks.
