Аннотация

OBJECTIVE: Cancer is a disease causing severe psychological distress in patients and family caregivers. This study aimed to investigate the effects on treatment adherence of psychological factors, such as anxiety, depression, and cancer stigma, of newly diagnosed cancer patients and their primary family caregivers.

MATERIALS AND METHODS: Psychological factors of 50 newly diagnosed cancer patients and 50 family caregivers were evaluated using the Beck Depression Inventory (BDI), Beck Anxiety Inventory (BAI) and the Questionnaire for Measuring Attitudes Toward Cancer (Cancer Stigma) (MATC)-Patient and Community versions (the patients completed the MATC-Patient version; while the family caregivers completed the MATC-Community version). In addition, the cancer patients were evaluated by a psychiatrist with a clinical interview. The patients were also prospectively monitored until the end of chemotherapy. Treatment adherence of the patients who completed their treatments was evaluated using the Medication Possession Ratio (MPR). The factors associated with treatment adherence were investigated using univariate and multivariate analyses.

RESULTS: 70% of the cancer patients had a high level of treatment adherence. Depression and anxiety symptoms and cancer stigma were more common in the group with low treatment adherence (p < 0.001). There were more patients with the diagnosis of major depressive disorder in the group with low treatment adherence (p < 0.001, RR = 5.692). Multivariate linear regression analysis showed that increase in the MATC-Community version score could be a potential risk factor for treatment non-adherence (p = 0.009).

CONCLUSION: Our study shows that psychological factors in newly diagnosed cancer patients and family caregivers, such as anxiety, depression, and cancer stigma, negatively affect treatment adherence. It was concluded that cancer stigma in family caregivers could be a potential risk factor for treatment non-adherence. We believe that our study will guide large-scale multicenter studies aiming to improve treatment adherence in cancer patients.
