OBJECTIVES: To map the global landscape of temporomandibular disorder (TMD) research across longitudinal cohorts, synthesize converging findings, and identify evidence gaps and priorities for future research.

DATA AND SOURCES: This scoping review followed Joanna Briggs Institute methodological guidance and the PRISMA extension for Scoping Reviews. PubMed, Scopus, and Web of Science Core Collection were searched from inception through May 2026.

STUDY SELECTION: From 3,543 records, 110 reports were included and mapped to 21 cohorts or data platforms across 11 countries.

CONCLUSIONS: Evidence across multiple cohorts indicates that psychosocial distress, sleep disturbance, parafunctional behaviors, and co-occurring pain conditions are associated with subsequent TMD onset, persistence, or related adverse outcomes, whereas occlusal characteristics generally show limited associations. These findings collectively support a biopsychosocial understanding of TMD. Important gaps remain: geographic and ethnic diversity is limited, coverage across the lifespan is uneven, TMD definitions are heterogeneous, and imaging and multi-omics data are scarce. Because study designs, phenotypes, and ascertainment methods varied substantially, these patterns should be interpreted as cross-cohort convergence rather than causal evidence. Future progress requires harmonized assessment methods, greater population diversity, validated prediction models, and prevention trials embedded within existing cohorts.

CLINICAL SIGNIFICANCE: Multiple cohorts identify psychosocial distress, sleep disturbance, parafunctional behaviors, and co-occurring pain conditions as candidate risk indicators for subsequent TMD onset or persistence. Developing and validating risk-stratification tools incorporating these domains may help primary care dentists identify patients who warrant closer follow-up before substantial functional impairment develops.
