AIMS: The Irish National Orthopaedic Register (INOR) was established to monitor total hip (THA) and total knee arthroplasty (TKA) practice, implant utilisation and patient outcomes in Ireland following the recall of the Articular Surface Replacement (ASR) metal-on-metal implant. This study presents the first decade of INOR data (2015-2024) from participating hospitals, evaluating patterns in arthroplasty activity, implant fixation, patient-reported outcome measures (PROMs) and clinical outcomes.

METHODS: A retrospective descriptive analysis of aggregated data from the INOR National Report (2015-2024) and accompanying appendices was performed, including demographics, procedural activity, implant utilisation, fixation strategies, PROMs (Oxford Hip Score [OHS], Oxford Knee Score [OKS], EQ-5D-5L) and clinical outcomes (30-day surgical site infection, dislocation, periprosthetic fracture, venous thromboembolism, mortality, and one-year revision).

RESULTS: A total of 43,291 arthroplasty procedures were recorded across 17 participating hospitals, including 22,249 primary THAs (51%), 18,102 primary TKAs (42%), 1,864 revision THAs (4%) and 1,076 revision TKAs (3%). Annual volume increased from 841 cases in 2015 to 10,266 in 2024. Cementless fixation in primary THA increased from 43% in 2015 to 71% in 2024. Mean OHS improved from 17.9 preoperatively to 44.0, and mean OKS from 19.5 to 40.5, at five years. Mean EQ-5D-5L improved from 0.265 to 0.818 following THA and from 0.364 to 0.793 following TKA at five years. The 30-day SSI rate was 0.5% for both THA and TKA; one-year revision occurred in 1.0% of THAs and 0.8% of TKAs, and 30-day mortality was 0.1% overall.

CONCLUSION: During its first decade, INOR expanded its capture of arthroplasty procedures and generated data on implant utilisation, PROMs and clinical outcomes among participating hospitals. Because of phased onboarding and incomplete national coverage, these findings reflect practice at participating institutions rather than a complete national picture, and revision rate estimates should be interpreted cautiously given incomplete denominator capture. As participation grows, INOR is positioned to support implant surveillance, quality improvement and evidence-based arthroplasty practice in Ireland.
