BACKGROUND: Elevated non-fasting triglyceride (TG) levels may be associated with increased risk of cardiovascular disease (CVD); however, the best way to measure TG values as an independent risk factor for CVD remains unclear. The aim of this study was to investigate whether the running average value of non-fasting serum TG (Ra-TG) measured after percutaneous coronary intervention (PCI) could be associated with the occurrence of subsequent cardiovascular events.

METHODS: The clinical records of 1761 patients who underwent successful PCI were retrospectively examined. Ra-TG values were calculated by adding consecutive non-fasting serum TG values and dividing them by the number of measurements. Based on the Ra-TG value during the observation period of 10 years after PCI, the patients were divided into 3 groups: Ra-TG ≥175, 115-175, and < 115 mg/dL. The incidence of major adverse cardiac events (MACEs; coronary-related deaths, nonfatal myocardial infarction, and new coronary stenosis of non-culprit lesions) was investigated in these groups. The associations between the incidence of MACEs and various risk factors were also investigated.

RESULTS: Kaplan-Meier analysis demonstrated that patients with a Ra-TG ≥175 mg/dL had a significantly higher incidence of MACEs than those in the other two groups. A multivariate analysis indicated that the Ra-TG value was significantly associated with the incidence of MACEs. Furthermore, regardless of patients' low-density lipoprotein cholesterol levels or histories of acute coronary syndrome, a Ra-TG value of more than 175 mg/dL was associated with a higher incidence of MACEs after PCI.

CONCLUSIONS: A high Ra-TG value is a predictor of increased recurrence of MACEs after PCI and long-term management of TG levels using Ra-TG values may be useful for preventing CVD.
