OBJECTIVES: This study aimed to conduct a meta-analysis to estimate the overall pooled prevalence of perinatal depressive symptom trajectories and their associated risk factors.

METHODS: A comprehensive literature search was conducted across PubMed, Web of Science, Embase, the Cochrane Library, CINAHL and PsycINFO on 19 October 2024. The search strategy used combinations of keywords related to perinatal depressive symptoms, trajectories, risk factors and analytic methods. Eligibility criteria were defined by population (adult perinatal women), exposure (at least one assessment during pregnancy and one within 12 months postpartum), outcomes (trajectory patterns, prevalence and associated risk factors), and study design (longitudinal trajectory-based studies). Meta-analyses were performed using Stata 17.

RESULTS: A total of 13,796 studies were identified, of which 22 (N = 47,650 participants) were included. All studies reported a persistent non-depressive trajectory, 15 reported a persistent depressive trajectory, 14 reported an increasing trajectory and 12 reported a decreasing trajectory. The pooled prevalence of persistent depressive trajectory was 9% (95% CI: 7%-12%), while the overall prevalence of increasing and decreasing trajectories ranges from 1.1% to 9.8% and 2.2% to 21.07%, respectively. Specific risk factors for persistent depressive trajectory included single status (OR = 1.68, 95% CI: 1.08-2.61) and low educational level (OR = 1.39, 95% CI: 1.02-1.89). Low income level (OR = 1.11, 95% CI: 1.07-1.16) and nulliparity (OR = 1.28, 95% CI: 1.12-1.47) were specific to the increasing trajectory, and stressful life events (OR = 1.40, 95% CI = 1.17-1.67) were associated only with the decreasing trajectory.

CONCLUSION: The burden of perinatal depressive symptoms on women is more variable. Identifying specific risk factors across depressive trajectories can provide healthcare professionals with a theoretical basis for individualized treatment of perinatal depression.
