Аннотация

Recent federal changes to the U.S. childhood immunization schedule have restructured the schedule-moving the COVID-19 vaccine and the hepatitis B birth dose for infants born to mothers testing negative to shared clinical decision-making, changing the recommended toddler formulation for varicella, and proposing a "consensus" core of 11 diseases routinely recommended for all children, even as the American Academy of Pediatrics, the World Health Organization's Strategic Advisory Group of Experts on Immunization, and other major professional societies continue to endorse the full complement of previously recommended immunizations. This commentary examines how nurses can serve as translational stewards of evidence-based immunization implementation-professionals who systematically translate, integrate, and defend the best available scientific evidence across clinical, community, and policy settings. Drawing on international evidence from the International Council of Nurses, the American Nurses Association (ANA) Code of Ethics, and behavioral prevention science, the commentary delineates a policy pathway encompassing four dimensions: (a) preserving relational trust as clinical infrastructure, (b) exercising independent evidence-based judgment anchored in multi-society consensus (i.e., the convergent recommendations of multiple leading professional organizations whose independent review processes have reached the same evidence-based conclusions), (c) building nurse-led immunization systems with expanded prescriptive authority, and (d) adopting practical front-line commitments including dual citation communication, interprofessional immunization charters, and nurse-led public engagement campaigns. The analysis addresses the roles of misinformation and disinformation in eroding vaccine confidence and positions nursing-as the most trusted profession in the United States for over two decades-as the stabilizing force that can hold immunization systems together when federal recommendations fracture.
