Downlook Technique for Superior (Head-End) Phacoemulsification in Deep-Set Eyes.
Аннотация
Deep-set eyes present a technical challenge during phacoemulsification cataract surgery because restricted access to the superior globe limits instrument maneuverability, particularly when surgery is performed through a superior (head-end) approach. Although the temporal approach is commonly preferred in such cases, a superior approach may be indicated in selected situations based on corneal pathology, astigmatism, or surgeon preference. We describe a simple "downlook technique" to facilitate superior (head-end) phacoemulsification in a 67-year-old man with bilateral Grade III-IV nuclear cataracts and deep-set eyes who underwent left-eye phacoemulsification with posterior chamber intraocular lens implantation under topical anesthesia. During surgery, the patient was instructed to maintain a downward gaze whenever superior access was required, thereby improving exposure of the superior corneal region and facilitating the creation of the main corneal incision, capsulorhexis, phacoemulsification, intraocular lens implantation, and main wound hydration. The remaining surgical steps were performed in the primary gaze when adequate maneuverability was available. The surgery was uneventful, and the patient achieved a best-corrected visual acuity of 6/6 on the first postoperative day with a clear cornea and a well-centered intraocular lens. The downlook technique provides a simple, practical method to improve superior globe exposure without altering the patient's body position, making it a useful adjunct during superior (head-end) phacoemulsification in patients with deep-set eyes, provided adequate patient cooperation is achieved.
