BACKGROUND

Repeat involvement with child protection and welfare services (CWS) is common, yet re-referrals and recurrence are often treated as interchangeable outcomes despite potentially reflecting different underlying processes. Evidence from Norway on the timing and predictors of repeat involvement remains limited.

OBJECTIVES

This study examines the timing and predictors of re-referrals and recurrence following a first referral to CWS, and assesses whether these outcomes represent distinct patterns of system involvement.

PARTICIPANTS AND

SETTING

Using national administrative data, a cohort of 214,905 children with a first referral to CWS between 2013 and 2022 was followed longitudinally.

METHODS

Time to-re referral and time to recurrence were analyzed using Cox proportional hazards models, adjusting for child characteristics, referral sources, assessed concerns, and initial decision outcomes. Adjusted cumulative risks were estimated at fixed time points.

RESULTS

Estimated cumulative risk showed that 17.7% of children were re-referred within one year of case closure, increasing to 49.5% within ten years. Recurrence was substantially less frequent, with an estimated cumulative risk of 4.6% at one year and 15.6% at ten years. Re-referral risk was associated with younger age, neglect, parental risk and health concerns, and closure decisions indicating limited service engagement. In contrast, recurrence was predicted differently by the case concerns and first decision outcomes, suggesting a more selective pathway to repeated substantiated concern.

CONCLUSIONS

Re-referrals and recurrence represent analytically and substantively distinct forms of repeat child protection involvement. Differentiating between these outcomes provides a clearer basis for interpreting repeat contact and supports the development of proportionate and differentiated service responses.
