OBJECTIVE

This study systematically mapped written mental health policies across all 50 U.S. state prison systems and the District of Columbia to assess their alignment with national standards issued by the National Commission on Correctional Health Care, American Correctional Association, and American Psychiatric Association.

HYPOTHESES: I anticipated (a) wide variability in policy alignment across states, (b) strong convergence on suicide prevention and intake screening, and (c) weaker attention to self-injurious behavior (SIB), restrictive housing safeguards for individuals with serious mental illness, staffing expectations, and continuity of care.

METHOD: Using a legal-epidemiologic policy surveillance design, I identified publicly available correctional policies and coded them against a nine-indicator framework derived from American Correctional Association, National Commission on Correctional Health Care, and American Psychiatric Association standards. Indicators included suicide prevention, intake screening, mortality review, SIB definitions and pathways, restrictive housing safeguards, inpatient transfer criteria, staffing expectations, and continuity-of-care provisions. Each jurisdiction received indicator-level scores (0-2) and a composite alignment score (0-100).

RESULTS

Policies varied widely in transparency and operational detail. All jurisdictions had some policy language addressing suicide prevention and intake screening, but full operational alignment varied across domains. Full alignment was most common for intake screening (94%) and inpatient or external psychiatric transfer criteria (80%) and least common for explicit staffing minima (4%) and stand-alone SIB definitions (25%). Composite alignment categories included high (23 jurisdictions), moderate (21), low (three), and very low (four). Qualitative analysis revealed differences in therapeutic versus custodial framing and implementation supports.

CONCLUSIONS

Written mental health policies in U.S. state prisons remain uneven in their operational specificity, particularly for SIB definitions and pathways, mortality review, restrictive housing safeguards, and explicit staffing minima. Strengthening these policy frameworks is critical to reducing harm, supporting staff decision making, and improving mental health outcomes in correctional settings.
