Аннотация

Testicular torsion is a urologic emergency caused by twisting of the spermatic cord, resulting in compromised testicular perfusion. Ischemia duration is the most important predictor of testicular salvage, and definitive surgical management should be rapidly pursued. While awaiting surgery, manual detorsion may restore blood flow and provide symptomatic relief. The testicular torsion traction technique (TTTT) is a bedside maneuver combining downward traction with external rotation of the affected testicle. A 14-year-old male presented to the emergency department with acute onset of 10/10 left testicular pain. Physical examination revealed variation in lie of the left testicle. The TTTT was performed, and symptoms immediately improved and then completely resolved. Doppler ultrasound revealed no evidence of torsion. Semi-elective bilateral orchidopexy performed one month later confirmed bilateral bell clapper deformity. This case further describes the TTTT for testicular detorsion and demonstrates its potential value as a temporizing measure, particularly in rural or low-resource settings with delayed access to ultrasound, surgery, or tertiary care.
