Аннотация

OBJECTIVE: We compared outcomes across common cancer types at an NCI-designated Comprehensive Cancer Center (NCI-CCC) (City of Hope) vs the U.S. Surveillance, Epidemiology, and End Results (SEER) national database. Cancer Centers offer a comprehensive approach to prevention, diagnosis, and treatment through tertiary services and by providing coordinated, personalized care.

METHODS: Propensity score matching (PSM) methods were employed in a 1:5 ratio for patients with incident cancers from 2004 to 2020 treated at COH vs reported in SEER.

RESULTS: The COH cohort had significantly superior overall survival (OS) compared with the SEER cohort across all examined cancer types after multivariable adjustment for key baseline characteristics and in the PSM analyses. For the PSM analyses: non-small cell lung cancer (NSCLC) hazard ratio (HR) 0.73, breast cancer HR 0.83, prostate cancer HR 0.62, colorectal cancer HR 0.74, pancreatic cancer HR 0.75, acute myeloid leukemia HR 0.79, acute lymphoid leukemia HR 0.77, and multiple myeloma HR 0.70 (all P < .001). Notable findings included substantially lower mortality risk for patients with any of the studied advanced solid cancers.

CONCLUSION: We found that cancer survival benefits were associated with patient care at an individual NCI-CCC employing state-of-the-art therapies and modern care. This work also highlights an opportunity to make adjusted survival outcomes more accessible to patients, complementing other types of comparative data now available. Further research could help to identify the drivers of observed survival advantages and promote the development of better guidelines and care models to improve outcomes for all patients.
