PURPOSE: This study aimed to evaluate the safety profile and clinical efficacy of the VIA360™ Surgical System (VIA360) for ab-interno canaloplasty in the management of glaucoma.
METHODS: This retrospective, noncomparative chart review included 64 eyes undergoing standalone VIA360 ABiC (VIA360-Alone) or VIA360 combined with cataract surgery (VIA360-Cataract) between March 13, 2025, and January 7, 2026. Surgical success was defined as ≥20% intraocular pressure (IOP) reduction at the last follow-up without surgical reintervention. Changes in IOP, glaucoma medications, and best-corrected visual acuity (BCVA) were evaluated through 6 months.
RESULTS: At 6 months, mean IOP decreased from 18.46 to 14.07 mmHg (-4.39 mmHg; -23.78%; P < 0.0001). In subgroup analysis, VIA360-Cataract eyes showed a reduction from 17.61 to 13.48 mmHg (-4.13 mmHg; -23.45%; P = 0.0011), while VIA360-Alone eyes decreased from 20.61 to 16.15 mmHg (-4.46 mmHg; -21.69%; P = 0.0549). Glaucoma medication use also decreased from 1.46 to 1.13 medications (-22.6%; P = 0.1331) overall at 6 months, with greater reductions observed in the combined group compared with standalone cases. Surgical success was achieved in 15 of 29 eyes (51.72%) with available follow-up. No intraoperative complications or vision-threatening adverse events were observed.
CONCLUSION: These findings suggest that VIA360 provides clinically meaningful IOP reduction with an excellent safety profile in early postoperative outcomes, demonstrating greater consistency when performed in conjunction with cataract surgery. Larger prospective studies are warranted to further define long-term durability.
