Аннотация

OBJECTIVES

To evaluate vertical bone height gain, implant stability, and osseointegration using allogenic bone ring grafts with simultaneous implant placement in the posterior mandible.

MATERIALS AND

METHODS

A prospective clinical study was conducted on patients with vertical ridge deficiencies in the posterior mandible. Each patient received an allogenic bone ring graft in conjunction with immediate implant placement. Clinical parameters-including signs of infection, soft tissue inflammation, postoperative pain, and neurosensory changes-were evaluated at 1 day, 1 week, 2 weeks, 3 weeks, and 4 weeks post-surgery. Implant stability was measured using the Implant Stability Test (IST) at both time points. Vertical bone gain was assessed using radiographic measurements (Cone beam computed tomography) immediate postoperatively and at six months postoperatively. This clinical trial was registered at ClinicalTrials.gov (NCT07054658).

RESULTS

The mean IST value significantly increased from 72.40 ± 4.63 at the time of placement to 82.20 ± 2.20 at six months (p < .001), indicating enhanced implant stability. Radiographic evaluation showed a mean vertical bone gain of 3.87 ± 0.47 mm intraoperatively and 3.42 ± 0.30 mm after six months (p < .05), reflecting stable graft integration and remodeling. No persistent signs of neurosensory deficits were observed.

CONCLUSIONS

The use of allogenic bone rings for simultaneous vertical ridge augmentation and implant placement in the posterior mandible appears to be a predictable and minimally invasive technique. It offers favorable clinical outcomes without the need for donor site harvesting, thereby reducing surgical morbidity and overall treatment time.

CLINICAL RELEVANCE: This technique offers clinicians a one-stage, less invasive approach for managing vertical bone deficiencies in the posterior mandible, minimizing surgical morbidity while ensuring favorable functional outcomes.
