ETHNOPHARMACOLOGICAL RELEVANCE: Multidrug-resistant organisms (MDROs) pose a severe global health threat, significantly complicating the treatment of bacterial infections. Traditional Chinese medicine (TCM), with its long history and rich clinical experience in treating infectious diseases, offers a unique and promising approach to enhance anti-infective efficacy and overcome resistance mechanisms when combined with antibiotics. The combination represents a novel strategy to combat MDROs.

AIM OF THE STUDY: This review summarizes the efficacy of TCM (including active ingredients, herbs and prescriptions) combined with antibiotics against MDROs based on in vitro and clinical evidence, and outlines their advantages, challenges and future directions.

MATERIALS AND METHODS: The review consulted the electronic databases PubMed and Chinese National Knowledge Infrastructure. keywords used for the search included "herb," "TCM," "Chinese herbal medicine," "antibiotics," "mechanism," "clinical studies," "in vitro experiments," and their combinations. Relevant literature was collected from 2002 to 2025.

RESULTS: This review demonstrates that TCM-antibiotic combinations exhibit significant synergistic antibacterial effects, markedly enhancing clinical outcomes and effectively reducing antibiotic resistance. The primary therapeutic effects include overcoming bacterial resistance, reduced treatment duration, and lower incidence of adverse effects. However, the future research should also pay more attention to the action mechanisms and the relationship between dosage and efficacy for the reliability of the applications.

CONCLUSION: TCM-antibiotic combinations have promising clinical application value for addressing MDROs infections. By integrating the multi-target actions of TCM with the rapid bactericidal effects of antibiotics, this approach offers a synergistic solution that enhances clinical outcomes and mitigates the emergence of resistance. Future research should focus on translating these findings into clinical practice through rigorous mechanistic studies and well-designed clinical trials, ultimately aiming to develop optimized therapeutic protocols for the effective management of MDROs infections.
