Аннотация

BACKGROUND: Deep brain stimulation (DBS) has been investigated as a treatment for various neuropsychiatric disorders. We recently conducted a feasibility trial to assess the safety and efficacy of DBS in the subgenual cingulate region (SCG) and uncinate fasciculus (UF) for post-traumatic stress disorder (PTSD). In our study, we observed a 55 % reduction in Clinician-Administered PTSD Scale (CAPS) scores at six months. Here, we present long-term follow-up data on the patients included.

METHODS: Four patients with PTSD were treated with SCG/UF DBS at our center. Long-term outcome was measured using validated clinical scales and neuropsychological testing at 6, 12 and 24 months.

RESULTS: Three patients were classified as long-term responders (CAPS reduction ≥ 50 %), and one as a partial responder (CAPS reduction ≥ 30 %). When considering group average data, we found a significant and sustained decrease in CAPS scores over time (60 % at 24 months). This was associated with a 40 % decrease in depression scores and significant improvements in the global assessment of functioning, as well as in the work/school and home life/family responsibilities sub-scales of the Sheehan Disability Scale. No significant changes were observed in neuropsychological scales, except for an improvement in apathy scores in the long term. The side effects observed in our trial were similar to those reported in patients treated with SCG DBS for other conditions.

CONCLUSIONS: We demonstrate that long-term SCG/UF DBS is safe and effective for patients with PTSD. Further research involving a larger cohort of patients is required to corroborate these preliminary findings.
