INTRODUCTION AND IMPORTANCE: This report presents a case of acute serous macular detachment and intraretinal edema following Implantable Collamer Lens implantation in a 27-year-old female, emphasizing the importance of surgical timing and the cautious use of off-label drugs.
PRESENTATION OF CASE: A 27-year-old female underwent Implantable Collamer Lens implantation after prophylactic laser photocoagulation. At the conclusion of the surgery, intracameral cefuroxime, typically used in cataract surgeries, was administered off-label. On the first postoperative day, the patient developed acute serous macular detachment and intraretinal edema in the left eye. Through targeted medical interventions, complete resolution of the retinal changes was achieved, and the patient regained normal vision at the three-month follow-up.
DISCUSSION: Laser photocoagulation may have contributed to macular detachment, as post-laser vitreoretinal traction can persist for months. The off-label use of intracameral cefuroxime also requires caution due to potential postoperative complications.
CONCLUSION: This case highlights the importance of careful evaluation of surgical timing after laser treatments and cautious use of off-labelintracameral medications. Surgeons should consider delaying surgery for at least two months after a re-examination of retinal status post-laser treatment to reduce the risk of complications.
