Evaluation of postextraction wound healing of immediate complete dentures using a modified inflammation proliferation remodeling scale following platelet-rich fibrin and xenograft application: A randomized clinical trial.
Аннотация
STATEMENT OF PROBLEM: Immediate denture insertion after dental extraction results in problems with socket healing, negatively affecting patient responses. Platelet-rich fibrin (PRF) and xenografts have been introduced to overcome such healing problems, but their clinical outcomes require further study.
PURPOSE: The purpose of this randomized clinical study was to assess postextraction healing and correlate it with bone volume changes during immediate denture insertion after socket treatment using PRF and xenografts.
MATERIAL AND
METHODS
This study included 24 randomly distributed participants; group RID received removable immediate dentures without socket treatment after extraction, and group PRF+XG received PRF and xenograft socket treatment before denture insertion. Postextraction wound healing was evaluated by using the inflammation proliferation remodeling (IPR) scale, and alveolar bone volume changes were assessed by using cone beam computed tomography (CBCT). Independent t test and linear regression analysis were conducted to assess relationships of the studied groups. The Pearson correlation coefficient was used to evaluate the direction and strength of the linear relationship between IPR change and bone volume change (α=.05).
RESULTS
Group PRF+XG reported higher inflammation phase scores and significantly enhanced healing with minimal IPR scores during the proliferation and remodeling phases (P=.01 and P=.03), together with significantly lower bone resorption for the 0- to 6-month interval (P=.01). The promoted healing in group PRF+XG was analyzed by using a steeper healing slope (-2.18) and greater statistical fit (R2=0.67), with a negative Pearson correlation coefficient between IPR scores and bone volume changes (-0.24), reporting that better soft tissue healing correlated with bone healing.
CONCLUSIONS
Combining PRF and xenograft treatment in a recent extraction socket before immediate denture insertion enhanced wound healing and minimized bone volume loss.
