Beyond new pills: integrative strategies to overcome multidrug-resistant bacteria.

The relentless global spread of multidrug-resistant (MDR) bacteria has outpaced the discovery of new antimicrobials, threatening to reverse decades of medical progress. The diminishing efficacy of traditional antibiotics underscores the urgent need for transformative, integrative strategies that combine innovative therapeutics, precision diagnostics, advanced delivery platforms and global policy interventions. This review examines recent advances and emerging technologies designed to overcome MDR pathogens. We synthesize developments in next-generation antibiotics, bacteriophage and Clustered Regularly Interspaced Short Palindromic Repeats (CRISPR) based therapies, antimicrobial peptides (AMPs), nanotechnology-enabled delivery systems, rapid molecular diagnostics and artificial intelligence (AI)-driven drug discovery. We also assess host-directed approaches, anti-virulence strategies and biofilm disruption techniques that complement traditional antibacterial interventions. Finally, we address the implementation science underlying stewardship, surveillance, and policy reforms required to sustain innovation and equitable access. We argue that the path forward will depend not on a single discovery but on a convergent ecosystem of biological, technological and policy innovations that together can outmaneuvre bacterial evolution.
