This study aims to assess the predictive effectiveness of color Doppler ultrasound (CDUS) versus electrocardiogram (ECG) in the early detection of ventricular remodeling (EVR) following acute myocardial infarction (AMI). This retrospective observational study included 306 patients diagnosed with AMI who were treated at our hospital between January 2023 and May 2024. Clinical and imaging data were retrospectively collected from medical records. Patients were divided into the EVR group and the control group according to whether EVR occurred within 72 hours of admission. Both groups underwent CDUS and ECG examinations during hospitalization. Baseline characteristics and laboratory parameters were compared between the 2 groups. Multivariate logistic regression analysis was performed to identify independent predictors of EVR, and receiver operating characteristic curve analysis was used to evaluate and compare the predictive performance of CDUS and ECG parameters. The diastolic blood pressure (DBP) of the EVR group was significantly lower, while low-density lipoprotein cholesterol, blood glucose, left ventricular ejection fraction (LVEF), LVEDD, left ventricular end-systolic volume (LVESV), QT interval, and QRS duration were notably higher compared to the control group (P < .05). Logistic regression revealed that DBP (OR = 0.916, 95% CI: 0.852-0.985, P = .017) and LVEF (OR = 1.279, 95% CI: 1.084-1.508, P < .001), LVESV (OR = 1.401, 95% CI: 1.213-1.617, P < .001), and QT interval (OR = 0.868, 95% CI: 0.757) were key predictors of EVR. The combined diagnostic accuracy for LVEF, left ventricular end-diastolic volume (LVEDV), and LVSV in predicting EVR showed an area under the curve of 0.975, sensitivity of 92.70%, and specificity of 91.90%, outperforming the QT interval (P < .001). DBP, LVEF, LVEDV, LVESV, and QT interval were identified as independent predictors of EVR. Among these, LVEF, LVEDV, and LVESV showed relatively higher predictive accuracy. Compared with ECG, CDUS demonstrated better overall performance in predicting EVR in AMI patients, suggesting its potential usefulness for early clinical assessment and individualized management.
