BACKGROUND: The 2025 American Thyroid Association (ATA) guideline introduced substantial updates, including separate categorization for follicular thyroid carcinoma (FTC) and revision of the traditional three-tiered system to a four-tiered system. In this study, we aimed to evaluate the predictive performance of the 2025 ATA risk stratification compared with the 2015 system in patients with FTC.
METHODS: A total of 347 patients with FTC who underwent thyroidectomy at a tertiary hospital in Seoul, Korea between August 1995 and April 2021 were evaluated. Disease-free survival (DFS) was analyzed using the Kaplan-Meier method, and proportions of variance explained (PVEs) were calculated to compare predictive accuracy.
RESULTS: Under the 2015 ATA system, 204, 71, and 72 patients were categorized as low, intermediate, and high risk, respectively. Applying the 2025 system, 39 of 204 low-risk patients (19.2%) were upstaged. Among 71 intermediate-risk patients, two (2.8%) were reclassified into the intermediate-high group, and four (5.6%) were upstaged to the high-risk group. The PVEs for predicting DFS were 19.99% for the 2015 system and 23.10% for the 2025 system.
CONCLUSIONS: The 2025 ATA risk stratification system resulted in frequent upstaging compared with the 2015 system and demonstrated improved predictive performance for DFS.
