Аннотация

Introduction: 75% of interventions performed due to infrarenal aortic aneurysms are endovascular. Compared to open surgery, this is associated with lower perioperative morbidity and mortality, shorter length of hospital stay (LOH), fewer early postoperative but more long-term complications. Objective: To evaluate the outcomes of elective bifurcated stentgraft implantations for infrarenal aorta and/or common iliac artery aneurysms, performed at the Department of Vascular Surgery, University of Pécs (Hungary) between 2010 and 2020. Method: In this retrospective analysis, data on demography, comorbidities, mortality, LOH, and intensive care requirement were collected. Intraoperative, early, and late complications were collected, and their associations were analyzed. Results: A total of 274 cases were included. Out of the 42 intraoperative complications (15.33%), 7 (2.56%) were endoleaks. 9 patients (3.28%) died within 30 days, 14 (5.11%) within the first year postoperatively. In the early period, 3 endoleaks (1.09%) and 1 aneurysm sac rupture (0.36%) developed. The most common stentgraft-specific complication was endoleak (n = 68, 24.82%). Intensive care requirement and LOH decreased significantly during the study period. The odds of early complications decreased significantly to one-third with diabetes mellitus. The odds of stentgraft-specific complications increased twice with hyperlipidemia and decreased to two-third with smoking. Conclusion: During the study period, intensive care requirement and LOH decreased significantly. Besides, we found an association of diabetes, smoking, and hyperlipidemia with complications. Decisions about an effective follow-up and treatment strategy of specific complications often requires individual decision-making due to the rapid development of devices and the small number of cases.
