Аннотация

BACKGROUND AND AIMS: Lower extremity peripheral artery disease (PAD) represents a major global health challenge, with significant disparities in disease burden across countries. This study aims to analyze the burden and epidemiological characteristics of PAD from 1990 to 2021, assess inequalities among nations, and project PAD burden trends over the next 15 years.

METHODS AND RESULTS: Using data from the 2021 Global Burden of Disease (GBD) study, we analyzed the prevalence, incidence, mortality, and disability-adjusted life years (DALYs) of PAD through Joinpoint regression, health inequality analysis, frontier analysis, and Bayesian age-period-cohort modeling. From 1990 to 2021, global PAD cases rose by 102.01 %, reaching 113.71 million, while the age-standardized prevalence rate (ASPR) declined by 12.33 %, accounting for population growth and aging. Females consistently exhibited higher prevalence and incidence. The burden was most pronounced in high socio-demographic index (SDI) countries, especially in terms of DALYs. Projections indicate rising ASPR and age-standardized incidence rates (ASIR) but declining age-standardized mortality rates (ASMR) and age-standardized DALYs rate (ASDR) over the next 15 years.

CONCLUSION: The global burden of PAD remains substantial and unequally distributed, with high-SDI countries facing growing challenges. Targeted interventions are essential to reduce disparities and mitigate future burden.
