62-year-old woman with type 2 diabetes mellitus (T2DM) and chronic kidney disease presented to the community pharmacy due to episodes of nocturnal hypoglycemia. Interventions were carried out including diabetes education, adjustment of the insulin regimen, and personalized dietary recommendations, which eliminated the hypoglycemic episodes, improved glycemic control, and enhanced the patient's quality of life. This case highlights the key role of community pharmacy in detecting and resolving medication-related problems in chronic patients.
