This commentary on a retrospective study of patients with HER2-positive biliary tract cancer highlights the importance of reflex and validated HER2 testing criteria, the discordance of next-generation sequencing and HER2 IHC, the potential prognostic and predictive role of HER2, and the necessity to define HER2 therapy resistance mechanisms.
