Unusual presentation of antrochoanal polyp: A case report of oropharyngeal involvement in a child.
Аннотация
INTRODUCTION AND
IMPORTANCE
Antrochoanal polyps are rare benign lesions arising from the maxillary sinus and extending into the nasopharynx. In exceptional cases, they extend into the oropharynx, causing symptoms such as dysphagia and voice changes. Recognizing this atypical presentation is crucial for timely diagnosis and management.
CASE REPORT: A 7-year-old boy presented with a 10-month history of left nasal obstruction, foreign body sensation in the throat, and chronic rhinorrhea. Recently, he developed dysphagia and voice changes. Examination revealed a soft, translucent mass behind the soft palate, displacing it anteriorly. Nasal endoscopy and computed tomography (CT) confirmed a left antrochoanal polyp extending into the oropharynx. The patient underwent functional endoscopic sinus surgery (FESS) with complete excision. Histopathologic analysis confirmed the diagnosis. Postoperatively, symptoms fully resolved, with no recurrence over 3 years.
CLINICAL DISCUSSION: Antrochoanal polyps typically present with nasal obstruction and rhinorrhea but rarely extend into the oropharynx. Such cases may lead to dysphagia, voice alterations, and potential airway compromise. CT imaging is essential for assessing the extent of the lesion. FESS remains the gold standard for treatment, ensuring complete removal and reducing recurrence risk.
CONCLUSION
This case highlights an unusual presentation of an antrochoanal polyp extending into the oropharynx. Clinicians should consider this diagnosis in pediatric patients with persistent nasal obstruction and upper airway symptoms. Early intervention with endoscopic surgery provides excellent outcomes.
