Аннотация

This case report presents the management and nine-year follow-up of a 9-year-old male with Larsen syndrome, severe cervical kyphosis (65°), and myelopathy. Diagnosis was confirmed clinically. Preoperative halo traction reduced kyphosis to 30°, followed by staged surgery under neuromonitoring: anterior cervical corpectomy with fibular strut graft and plating, then posterior fixation with pedicle screws from C2 to C6. Postoperative imaging confirmed spinal cord decompression and stable fusion. Neurological function improved significantly, with mJOA scores rising from 8/18 preoperatively to 18/18 at one year, sustained through nine years. Radiographs showed maintained correction and solid fusion without hardware complications. This report emphasises the complexity of cervical deformity in Larsen syndrome and supports combined anterior-posterior 360-degree fusion with preoperative traction for severe kyphosis and myelopathy. Long-term follow-up until skeletal maturity is critical to monitor deformity progression and adjacent segment integrity.
