BACKGROUND: The clinical characteristics of respiratory syncytial virus pneumonia (RSVP) in children with atopy were studied, and the predictive indicators of RSVP and prognosis were determined. This study aimed to explore the clinical characteristics and prognosis of RSVP in children with atopy and to determine the predictive indicators of RSVP.

METHODS: A retrospective cohort study was conducted involving the clinical and post-discharge follow-up data of 294 children with RSVP who were admitted to the Second Department of Respiratory Medicine of Xi'an Children's Hospital from January 2023 to December 2024. The atopic status of the children with RSVP was determined post-hospitalization. The children were divided into atopic (109 cases) and non-atopic groups (185 cases). The clinical characteristics of RSVP in atopic children and the risk factors for severe pneumonia were investigated.

RESULTS: The atopic group with atopy determined post-hospitalization had significantly higher rates of severe pneumonia (27.5% vs. 17.3%), severe cough (39.4% vs. 27.6%), shortness of breath (32.1% vs. 21.1%), pulmonary rales (40.3% vs. 27%), intrapulmonary complications (7.3% vs. 1.6%), oxygen therapy (18.3% vs. 8.7%), and hormone therapy (28.4% vs. 18.4%) compared to the non-atopic group. There were no differences in the laboratory measures between the two groups. The number of newly diagnosed asthma cases within 6 months after discharge (5.5% vs. 0.5%) was significantly different between the two groups (P<0.05). Multifactorial logistic regression analysis revealed that shortness of breath and pulmonary rales [odds ratio (OR) =4.402 and 3.583, respectively; P<0.05] were independent risk factors for severe pneumonia in children with atopy (P<0.05).

CONCLUSIONS: Children with atopy determined post-hospitalization and RSVP have a higher incidence of severe cough, shortness of breath, pulmonary rales, and intrapulmonary complications during hospitalization than children without atopy. Children with atopy and RSVP also have a higher rate of requiring oxygen and hormone therapy. Shortness of breath and pulmonary rales have good clinical predictive value for the occurrence of severe pneumonia in children with atopy. Moreover, the risk of asthma still exists in children with atopy after discharge, necessitating timely follow-up and early intervention.
