Analysis of the correlation between abdominal pain and long Meckel's diverticulitis on the mesenteric side: a case study in the context of mesenteric volvulus and internal hernias: a case report.
Аннотация
BACKGROUND
Meckel's diverticulum is a common congenital gastrointestinal malformation that affects approximately 2% of the population. While the majority of individuals with this condition remain asymptomatic, complications such as intestinal obstruction, hemorrhage, and inflammation can occur, leading to significant clinical implications. Despite its established prevalence, the pathophysiological mechanisms underlying Meckel's diverticulum and its associated complications are not well understood, presenting diagnostic challenges for medical professionals.
CASE PRESENTATION: This report describes a 30-year-old male of Han Chinese ethnicity who presented with persistent colicky pain in the lower abdomen, which began 5 hours prior to admission, accompanied by a cessation of gas passage and bowel movements. An enhanced computed tomography scan revealed mesenteric congestion and small bowel distension, raising suspicion for mesenteric volvulus or an intra-abdominal hernia. Laparoscopic exploration was performed owing to worsening abdominal pain, during which necrotic portions of the small intestine and a 25 cm-long giant Meckel's diverticulum were identified. The diverticulum was excised, and the patient had an uneventful postoperative recovery.
CONCLUSION
This case highlights the rarity of mesenteric volvulus resulting from Meckel's diverticulum and emphasizes the importance of considering this condition in patients presenting with bowel obstruction symptoms. It underscores the need for timely diagnosis and intervention to prevent severe complications. In addition, the study calls for further research to explore the complex mechanisms associated with Meckel's diverticulum and its complications, aiming to optimize treatment strategies and improve patient outcomes.
