BACKGROUND/OBJECTIVE: Postmenopausal women with Type 2 Diabetes Mellitus (T2DM) face heightened cardiometabolic risks due to the combined effects of estrogen decline and metabolic dysfunction. While aerobic and resistance exercises are effective in treating and preventing these complications, the optimal order of exercise in a combined session remains unclear for this vulnerable population. This study compared the execution order of a combined exercise session on glycemia, blood pressure (BP), and heart rate variability (HRV) in postmenopausal women with T2DM.

METHODS: A randomized crossover clinical trial enrolled 15 postmenopausal women with T2DM in three experimental conditions: (I) 30 min of aerobic exercise + 30 min of resistance exercise (AR); (II) Resistance exercise + aerobic exercise (RA); (III) Resting session (CON). Glycemia was assessed pre, mid, and immediately at the end of exercise, and 20, 40, and 60 min post-exercise. BP was assessed pre and at 20, 40, and 60 min post-exercise. HRV was assessed pre, at 10, 30, and 50 min post-exercise.

RESULTS: After aerobic exercise, the AR session demonstrated a lower blood glucose level than the CON session (p = 0.01). The AR session demonstrated a lower area under the glycemic curve during exercise compared to the RA and CON sessions (p < 0.05). Post-exercise, all sessions reduced glycemia (p < 0.05), with no difference among conditions (p > 0.05). Both exercise sessions reduced systolic BP (p < 0.05), however, only in the AR session was there a reduction in diastolic BP (p = 0.01) after exercise. Regardless of the order, exercise sessions reduced time domain indices of HRV (p < 0.05).

CONCLUSION: The AR order led to a greater reduction in glycemia during exercise and tended to be more effective in reducing BP after exercise. However, no differences were observed in the HRV response between the AR and RA sessions.
