The Unheard Patient: Active Listening as a Core Clinical Skill in Aesthetic Surgery.
Аннотация
Aesthetic surgery carries a disproportionate medicolegal burden attributable to communication failure, with unmet expectations accounting for 14.4% of malpractice claims in plastic surgery compared with 3.8% in other medical specialties. No existing framework in the aesthetic surgery literature addresses active listening as a structured, teachable clinical skill. A narrative review of the literature was conducted across MEDLINE/PubMed, Embase, PsycINFO, and Scopus (January 2010 to April 2026), with sources selected by discussion between two authors (O.F. and D.M.) against stated eligibility criteria. The literature consistently identifies attentiveness deficits, premature judgment, and failure to acknowledge patient emotion as proximal mechanisms linking surgeon behavior to complaints and litigation. These patterns map directly onto five discrete listening micro-skills: adopting a learning stance, suspending judgment, paraphrasing, inquiry, and acknowledgment. In aesthetic surgery specifically, active listening constitutes an ethical and conceptual precondition for valid informed consent, because consent in this specialty requires alignment on a subjective goal that cannot be clinically examined; it can only be heard. Active listening is not a soft skill peripheral to aesthetic surgical practice; it is its primary diagnostic instrument. A structured listening framework offers surgeons a teachable model for improving patient experience, reducing complaint risk, and supporting the validity of informed consent.
