OBJECTIVE: To investigate the predictive value of lactate/albumin ratio (LAR) combined with neutrophil/lymphocyte ratio (NLR) for the 28-day prognosis in patients with sepsis.

METHODS: A retrospective cohort study was conducted. Adult patients with sepsis admitted to Tianjin University Haihe Hospital from April 2023 to August 2025 were enrolled. The clinical data were collected, including gender, age, complications, and vital signs; lactate, blood gas analysis parameters, blood routine indexes, blood biochemical indexes within 24 hours of admission, as well as 28-day prognosis were recorded. Glasgow Coma Score (GCS), Sequential Organ Failure Assessment (SOFA) and Acute Physiology and Chronic Health Evaluation II (APACHE II) were calculated. Based on the survival status during the 28-day follow-up, the patients were divided into a survival group and a death group. Independent risk factors for prognosis in septic patients were identified using multivariate Logistic regression analysis, and receiver operator characteristic curve (ROC curve) was drawn to evaluate the predictive value of each factor for patient prognosis.

RESULTS: A total of 440 patients were finally included, among whom 242 survived and 198 dead. Compared with the survival group, the death group had lower oxygenation index, lymphocyte count (LYM), albumin, GCS score, and higher NLR, lactate, LAR, creatinine, D-dimer, SOFA score, and APACHE II score. The differences were statistically significant (all P<0.05). There were no significant differences in gender, age, complications, mean arterial pressure, pH, white blood cell count, neutrophil count, platelet count, packed cell volume, total bilirubin, sodium and potassium between the two groups (all P>0.05). Multivariate Logistic regression analysis showed that elevated NLR, LAR, SOFA score and APACHE II score were independent risk factors of 28-day mortality in sepsis patients, while elevated LYM was protective factor [NLR: odds ratio (OR)=1.015, 95% confidence interval (95%CI) was 1.003-1.028, P=0.014; LAR: OR=158.465, 95%CI was 13.988-1 795.246, P<0.001; SOFA score: OR=1.128, 95%CI was 1.034-1.230, P=0.007; APACHE II score: OR=1.060, 95%CI was 1.026-1.096, P<0.001; LYM: OR=0.601, 95%CI was 0.401-0.901, P=0.014]. ROC curve analysis showed that the AUCs of NLR, LAR, SOFA score, APACHE II score, and NLR+LAR for predicting 28-day prognosis of septic patients were 0.626, 0.686, 0.722, 0.717, and 0.717, respectively. There were no significant differences between the predictive value of the NLR+LAR combination and that of SOFA score (Z=0.164, P=0.870) or APACHE II score (Z=0.019, P=0.985).

CONCLUSIONS: NLR combined with LAR has predictive value for 28-day prognosis of septic patients, its predictive value is similar to that of SOFA score and APACHE II score, and the calculation process is convenient and simple, making it more suitable for clinical application.
