Аннотация

Atrial fibrillation (AF) is a progressive structural disease and a major cardiovascular concern. Traditionally, treatments focused mainly on rhythm control and stroke prevention. However, recent clinical guidelines emphasize a comprehensive approach that targets the underlying causes of atrial remodeling and systemic metabolic dysfunction. This review outlines current evidence for lifestyle interventions and the management of cardiovascular risk factors to improve patient outcomes. Lifestyle modifications are essential for the primary and secondary prevention of AF. Strict alcohol reduction, limiting intake to three or fewer drinks per week, decreases the risk of arrhythmia recurrence. In contrast, recent trials confirm that moderate caffeine consumption does not increase the risk of AF, meaning routine restriction is unnecessary. Regular moderate aerobic exercise is beneficial, although extreme endurance training may increase risk. Aggressive management of comorbidities is equally important. Obesity contributes to atrial fibrosis through the accumulation of epicardial adipose tissue. Weight loss programs achieving a 10% reduction can reverse disease progression. Hypertension is the most common risk factor, and strict blood pressure control significantly mitigates irreversible structural changes. Furthermore, novel pharmacotherapies for diabetes mellitus, such as sodium-glucose cotransporter 2 inhibitors and glucagon-like peptide-1 receptor agonists, provide substantial structural and electrophysiological benefits beyond glycemic control. Finally, the identification and treatment of obstructive sleep apnea with continuous positive airway pressure improves the success rates of medical therapies and catheter ablation. Ultimately, the successful treatment of AF requires more than medical procedures alone. A multidisciplinary approach addressing comprehensive lifestyle changes and systemic comorbidities is necessary to achieve stable clinical outcomes.
