OBJECTIVE: Life-threatening complications of severe preeclampsia can occur even after delivery. We present a case of a patient with severe preeclampsia whose disease progressed after delivery complicated with HELLP syndrome and postpartum hemorrhage (PPH).
CASE REPORT: A 36-year-old woman with severe preeclampsia underwent an emergent cesarean due to fetal distress. After delivery, she encountered exacerbated severe preeclampsia with HELLP syndrome, later leading to PPH and disseminated intravascular coagulation. Hemorrhage and coagulopathy caused subsequent intraabdominal hematoma and multiple organs injuries, which well recovered after blood transfusion and surgical evacuation of hematoma. She was discharged under relative stable condition.
CONCLUSION: Since fatal complications of severe preeclampsia may appear after delivery, optimal assessment and close postoperative monitoring are crucial in patients with severe preeclampsia to ensure maternal outcomes.
