Аннотация
Approximately 10% to 15% of patients with ulcerative colitis (UC) develop Crohn's-like disease of the pouch (CLDP) after restorative proctocolectomy with ileal pouch anal anastomosis∗.1,2 There are no therapies approved specifically for CLDP, and current guidelines recommend the use of advanced therapies approved more broadly for UC and Crohn's disease (CD) with reported remission rates of 40% to 60%.3 Comparative effectiveness data for these off-label therapies in CLDP is scarce, and optimal positioning is unknown. The head-to-head SEQUENCE trial demonstrated that risankizumab was noninferior to ustekinumab for clinical remission at week 24 and superior for endoscopic remission at week 48 in biologic-exposed patients with moderate-to-severe CD.4 Whether these results extend to patients with CLDP is unclear.
